GLP-1 Plateau at Month 6: What Actually Causes It

The first few months on a GLP-1 medication often feel remarkable. The weight comes off steadily. Clothes fit better. The scale rewards you almost every week. Then, somewhere around month four to six, the progress slows. The weekly drops get smaller. Eventually the scale stops moving for weeks at a time. Many patients reach this point convinced the drug has stopped working and something is wrong.

In most cases, nothing is wrong. The plateau is exactly what the trial data predicted. The body is doing what bodies do when they lose a significant amount of weight: it fights back. Understanding why this happens, and what actually moves the needle when it does, turns a moment of panic into a manageable phase.

What the Trial Curves Actually Look Like

The weight loss curves from the major GLP-1 trials are remarkably consistent in shape. They are not straight lines. They start steep and gradually flatten.

In the STEP 1 trial of semaglutide (Wilding and colleagues, 2021), weight loss was rapid in the early months, slowed progressively, and approached a plateau roughly around the second half of the treatment period before leveling off. The SURMOUNT-1 trial of tirzepatide (Jastreboff and colleagues, 2022) showed the same general pattern, with faster early loss giving way to a flattening curve over time.

This shape is the rule, not the exception. The curve flattens because the rate of weight loss naturally declines as you lose weight, not because the medication loses potency. A patient who expects linear progress (the same number of pounds every month indefinitely) is expecting something the trial data never showed. The slope was always going to flatten. Knowing that in advance changes how you interpret the stall.

Why the Body Resists Continued Weight Loss

When you lose a meaningful amount of weight, your body activates several mechanisms that resist further loss. This is not a malfunction. It is an ancient survival system that evolved to protect against starvation, and it does not know the difference between a famine and an intentional weight loss effort.

Leptin drops. Leptin is a hormone produced by fat tissue that signals satiety and energy availability to the brain. As you lose fat, leptin falls, and the brain interprets the lower level as a signal to increase hunger and conserve energy.

Ghrelin rebounds. Ghrelin is the hormone that drives hunger. Weight loss tends to increase ghrelin, pushing appetite back up. GLP-1 medications blunt this effect, which is part of why they work, but they do not eliminate it entirely.

NEAT declines. NEAT stands for non-exercise activity thermogenesis, which is all the calories you burn through daily movement that is not formal exercise (fidgeting, walking, standing, gesturing). As you lose weight, NEAT tends to drop, often without you noticing. You move a little less, and the energy you burn falls.

Thyroid output adjusts. Weight loss and caloric restriction can down-regulate thyroid hormone activity, lowering metabolic rate. This is part of the body’s energy conservation response.

Metabolic adaptation accumulates. The overall effect of these changes is that your body burns fewer calories at a lower weight than you would predict from the weight loss alone. Research on metabolic adaptation, including the well-known follow-up studies of Biggest Loser contestants by Hall and colleagues, demonstrated that this adaptation is real, substantial, and can persist for a long time. The body defends its weight more aggressively than most people expect.

The GLP-1 medication counteracts some of these mechanisms, particularly the appetite-related ones, which is why it produces more weight loss than dieting alone. But it does not fully override the body’s energy conservation response. The plateau is the point where the medication’s effect and the body’s resistance reach a balance.

Plateau Versus Set Point

There is an important distinction between hitting a temporary plateau and reaching the weight your body is prepared to settle at for now.

A temporary plateau is a stall that can be pushed through with the right adjustments. The body has adapted, but there is more fat to lose and levers remain to pull.

Reaching a sustainable lower weight is different. At some point, your body composition reaches a level that is appropriate and maintainable for your current situation. Continuing to push for more loss past that point can start costing you muscle and quality of life for diminishing returns. Part of the clinical judgment is recognizing which situation you are in. Not every plateau needs to be broken. Some plateaus are the goal.

Levers That Push Through a Plateau

When a plateau is genuinely temporary and there is more fat to lose, several clinical levers can restart progress.

Dose escalation. If you are not yet at the maintenance dose, moving up within the labeled range (under clinical supervision) often restarts weight loss. The plateau sometimes simply reflects that the current dose has reached the limit of its effect for you.

Switching molecules. Semaglutide and tirzepatide work through related but different mechanisms. Tirzepatide acts on two receptor pathways rather than one. A patient who plateaus on semaglutide sometimes responds to a switch to tirzepatide, and where available, newer agents like retatrutide add a third mechanism. Addressing insulin resistance directly. If underlying insulin resistance is driving the metabolic picture, targeting it through diet composition, training, and sometimes additional interventions can help the medication work better.

Sleep and stress. Poor sleep and chronic stress raise cortisol, which works against fat loss and promotes visceral fat storage. Resistance training and protein recalibration. Adding or increasing resistance training raises energy expenditure and protects muscle, which protects metabolic rate. Recalibrating protein intake to the new lower body weight supports the same goal. This connects directly to the muscle preservation framework in the post on GLP-1 muscle loss.

Addressing co-occurring hormone imbalances. A plateau is sometimes a signal of an unaddressed hormone problem. Low testosterone in men and the hormonal shifts of perimenopause in women both affect body composition and can stall weight loss. Treating the hormonal driver alongside the GLP-1 sometimes restarts progress that the medication alone could not achieve.

The Diet Refeed Question

Some patients benefit from structured diet adjustments around a plateau, including periods of eating at maintenance calories rather than in a deficit. The rationale is that continuous aggressive restriction can deepen metabolic adaptation, and brief periods at maintenance may help reduce that adaptation before resuming a deficit. The evidence here is mixed and the approach should be individualized, but it is part of the toolkit for some patients. Protein should stay high throughout regardless of overall calorie level.

What Happens When You Stop

The plateau conversation eventually leads to the maintenance question. What happens when you stop the medication?

The honest answer is that weight regain is common after GLP-1 discontinuation if there is no structured plan, because the same metabolic adaptations that caused the plateau are still present, and the appetite suppression that the drug provided is gone. This is why the off-ramp matters as much as the on-ramp. A well-structured program plans for maintenance from the beginning: building muscle to protect metabolic rate, establishing sustainable eating patterns, and deciding whether a lower maintenance dose or a gradual taper fits the patient’s situation. The goal is to hold the results, not just achieve them temporarily.
